Last week, CMS released the CY 2023 Home Health Payment Proposed Rule.  The news is not cheery.  In fact, as Bill Dombi, President & CEO of the National Association for Home Care & Hospice stated, “CMS has declared war on home health care”.  During the 60-day comment period which will end on August 16, 2022, individuals will need to advocate against the proposed national standardized 30-day base rate payment reduction of $126.88.  During this time of unprecedented inflation with soaring labor and gas prices, this decrease in payment presents major financial challenges to the home health industry.  The proposed rule will also recalibrate all 432 case-mix weights and all LUPA thresholds.  A majority of the case-mix weights, 238 groups, may decrease as much as 5% while the remaining groups will experience a positive or no increase.  CMS is also proposing OASIS collection on all patients in an effort to collect data.  This could result in increased burden for home health agencies who service patients who are not included in the OASIS collection requirement. Other less ominous proposed changes include three new G-codes to collect data on telecommunications.  Unfortunately, telecommunications will continue to be non-billable.  Finally, a change in the HH Value Based Purchasing baseline year is proposed to move from 2019 to 2022 for existing agencies.  More details and additional rule changes will be included on a free CY 2023 Home Health Payment Proposed Rule summary which will be available from MAC Legacy in a few days.  In the meantime, we can be thankful it’s Friday.
